Annual Meeting Partnership for a Healthy Community
The Annual Meeting for the Partnership for a Healthy Community will be held February 12, 2019, from 8:00 AM to 12:30 PM at the Countryside Banquet Facility in Washington, IL.
The Partnership for a Healthy Community is a community-driven effort to improve health and wellness in Central Illinois. Multiple organizations, sectors, and the public participate in population health planning to identify and prioritize health and quality of life issues, map and leverage community resources, and form effective partnerships to implement health improvement strategies in Peoria, Tazewell, and Woodford Counties.
Using actionable data to identify health needs and priorities, including those related to health disparities, health inequities, and the social determinants of health, the Partnership for a Healthy Community developed the 2016-2019 Community Health Improvement Plan.
The Annual Meeting will focus on the progress of the initiatives that address the four identified priority health issues in Central Illinois:

- Behavioral Health - mental health, substance abuse, suicide and social-emotional issues
- Healthy Eating/Active Living – nutrition, physical activity, obesity
- Breast Cancer and Lung Cancer – health screenings, genetics, tobacco, radon
- Reproductive Health – STIs, low birth weight babies, teen births
